White Stag Crew 122: Registration Forms

Below are links to the required forms, in print-ready Adobe Acrobat .pdf format; the registration forms are also available as editable word processing documents. (If you use word processing software other than Microsoft Word, you may find the .rtf file easier to use than the .doc file.)

The Class 3 Medical Form must  be signed by a doctor, and we need that form with the doctor’s original signature. Likewise, we need parent/guardinan original signatures and initials on the Authorization & Consent Form.

If your child needs to take a medication during his/her week at White Stag, please also complete the Medication Form, and include that with your other forms. Thank you.
